TB HIV Care (THC), in existence since 1929, is an internationally recognized non-profit organisation. Purpose of the position:
The main duty of this cadre is to track and trace patients from different geographical areas as per facility line lists to ensure retention in care.
Minimum Requirements :
Grade 12
Valid 10-day HIV AIDS counselling certificate from recognised service provider (will be advantageous)
One experience working in HIV and TB programmes
Proficiency in English and isiXhosa languages
Duties and Responsibilities :
Track and trace patients from different geographical areas as per facility line lists
Report back to the facility using the tracing tool/register and other recommended paper-based tools
Encourage and support disclosure of relevant information
Conduct individual and group health education using the teaching aides provided
Promote correct and consistent condom use.
Keep abreast of the latest developments in HIV/AIDS/TB/STI prevention
Support community outreach activities to bring return patients to treatment
Work as part of a multi-disciplinary team and maintain a high standard of professionalism in all interactions.
Refer service users to the Nurse Clinician or Enrolled Nurses for further tests where necessary
Comply with relevant legislation and organizational policies and procedures
Perform any other duties assigned by the supervisor
Demonstrate empathy and a non-judgmental attitude toward service users
